Patient’s Discharge

Overview

Our nurses and other healthcare professionals will ensure a smooth discharge process before you leave the hospital. 

The following items will be prepared for you before you leave:

  1. Discharge Summary
  2. Discharge Medication (if any)
  3. Follow-up appointment (if any)
  4. Lab / Radiology reports (if any)
  5. Hospitalization leave or Medical Certification (if any)

If you need other reports or supporting documents regarding your stay at Crawfurd hospital, our friendly officers will be able to assist you.

Before you leave

Our dedicated staff will arrange other referrals to provide holistic care to you, if necessary.

 

Check Out Time

Hospital check-out time will be 1200 hr.

Late checkout can be arranged with additional charges.

Transport and personalised chauffer can be arranged.
